If your health insurance is provided by a major carrier, there’s a good chance that your treatment could be covered at The Farley Center. To find out what your policy covers and to explore your treatment options, please complete our simple verification form. You will need to have the following information ready:

Personal Information:

Provide your full name, contact details, date of birth, and any other relevant identifying information.

Insurance Information:

Enter details from your health insurance card, including the name of the insurance company, your policy number, group number, and any other pertinent information.

Please note that The Farley Center currently does not accept Medicaid or Medicare insurance.

Additionally, in partnership with the Department of Veterans Affairs Community Care Network, The Farley Center offers the Tactical Recovery Program designed specifically for active duty members, military Veterans, and their families.
